ANuChance; I came across your post and I am in network. There are no upfront fees from the surgical office. The bariatric center does have a one time fee of upto $250 which covers the nutritionist and bariatric mental health eval. After that, everything is billed to your insurance and you are just responsible for copays and deductables which is the same as seeing any other doctor. We have never had anything like the huge fees charged at other programs. Kristen, At the Norton affiliated hospitals, downtown and at Suburban, we are looking at the surgical treatment for adolescents, but at this point, we do not perform bariatric surgery at either hospital on anyone less than 18 years old. I know of a dedicated pediatric bariatric program in Cincinnati. I hope this is helpful. Lydia, the way I would handle it is to perform your gallbladder surgery first since you are having symptoms from that. I too, would not perform gallbladder and lap-band surgery at the same operation for fear of infection. It depends. I think that Medicaid requires 6 months documented physician supervised Medical Weight Management. But i am not completely sure. I know a few months ago we got a letter from medicaid reporting the new guidelines whi*****lude more requirements than their previous guidelines. Dr. Shina's office might know. I know we have that documentation of requirements at my office, but I don't have that in front of me right now. Theresa, Keep up the fight. Any luck with your employer? I do participate in-network in most insurance plans. What that means is a patient is responsible for their copays at each office visit and their deductable. Many patients only pay copays and have no additional costs because their deductable has already been met. With any surgery and any size deductable (even big deductables), you can be pretty sure you will meet the deductable for that year. If you go with an out-of-network physician, most times there is an additional out-of-network deductable and an in-network deductable with your insurance in addition to whatever fees the out-of-network physician chooses to charge. Let me give an example; If the charge for something is $10,000, and the insurance pays the in-network doctor $500 and says that is the allowable, then that is all the in-network doctor gets. The out-of-network doctor may get the same $500 from the insurance, but the patient may be responsible for the entire difference or $9,500 in this example. The reason for this potentially large extra cost to the patient is that the out-of-network doctor has no contract with the insurance company to limit the reimbursement. Where the in-network doctor does have a contract with the insurance company and has already 'agreed' to accept what the insurance company pays. I know this is a very confusing topic and even after running my own practice, I still have a lot to learn. I feel patients need to inform themselves of these issues ahead of time, so there are no financial surprises down the road. I hope this answers the questions. Cindy, Medicaid policy has changed in the last 6 or 8 months. They do now require 6 month Medical Weight Management, where they didn't use to. There are not many bariatric surgery doctors in the state that participate with Medicaid, and for that matter, Medicare either. In Louisville, I believe most of the Norton affiliated surgeons practices participates with Medicaid and Medicare, including my solo practice @ Suburban hospital Dr. Shina's solo practice @ Suburban hospital and the USA surgeons @ the downtown Norton Hospital (Allen,Larson and Rodgriquez). There is a medicine -- called DEVROM. This is used alot in the BPD-DS (biliopancreatic diversion-duodenal switch) population and not as much in gastric bypass patients. It shouldn't be an issue at all with Lap-Band patients. Basically it is a bowel deoderant. It doesn't reduce the amount of gas, it is supposed to cover up the odor. I have only seen it online. We used to have a few samples, but not anymore. I include the web link below. Good luck!!! Also, you may need to avoid certain foods -- the same foods colostomy patients should avoid. You may get more air in your system if you chew gum, talk fast, or drink thru a straw. Cheri, Weight gain may to be approached from several pointsl; surgical, mental and nutrition. To evaluate surgical correctable causes of weight gain is easy -- generally an endoscope (EGD) will tell if the pouch looks ok. The others are more tricky. There are many emotional or mental blocks to proper eating and many times these need to be addressed before successful weight loss can continue. Gene Hedden has done many of the bariatric psych evaluations for my patients and he runs a support group (separate from the one at the hospital) just addressing mental health in post-bariatric surgical patients. Nutritional problems can be evaluated by seeing the bariatric program nutritionist -- and many times 'relearning' the proper diet can help and many times keeping a food log can help you be honest with yourself about what you are really eating. These are tools that I frequently use in postop patients, not just pre-op. Having regular support can make a difference too. It has been shown that regular participation in support groups improves the success after weight loss surgery. I think the support obtained here at obesityhelp.com is significant, but i think it is even more powerful to meet others face to face who are experiencing the same issues. Now that our program has merged with the Norton Bariatric Center, we have support groups the second and fourth Tuesdays of every month. I hope this is some help to you. The most important point is that obesity is a lifelong disease that surgery helps to tame. Katherine, The costs are about the same out-of-pocket as before RightWeigh left the program, maybe even a little less now depending on your insurance policies co-pay. Through the Norton Bariatric Center, I believe there is $100 program fee, and then the nutritionist and psychological evaluations are billed to the insurance. So, there may be a co-pay with both of those 'providers'. After the process is complete with the bariatric center, then a consult with the surgeon is arranged, and only the typical copay is due. Kathy, The support groups are still the 4th Tuesday of each month at 730pm at Suburban Hospital in Plaza III. There is an informational seminar at 600pm at the same location. This coming Tuesday the 22nd, I will be presenting the informational seminar. From the office of Dr. David Geller, We get this question asked alot, so I thought I would post this here. We do participate IN-NETWORK with most insurance policies. Typically, by staying in network, patients are only responsible for the standard co-pay and deductable. We are aware that there are programs that typically are OUT-OF-NETWORK, also known as NON-PAR or NON-PARTICIPATING or OFF-NETWORK. Some insurance policies simply do not provide any coverage for any doctor or hospital that is not in their network. For the policies that do allow for coverage for out of network care, typically there is a separate 'OUT-OF NETWORK' deductable to pay and higher copays. The arrangement of being out of network does usually pay the doctor/provider more, but this puts a much higher financial penalty on the patient. We are aware of programs that charge the patient high charges, in the thousands, for amounts that patients would not be responsible for at all by staying in network. I hope this clears this up. Dealing with insurance policies and always changing requirements for surgery can be a consusing issue. Please call if there are any questions. Thank you for your comments. Let me explain the change to our program. The hospital has ended its contract with RightWeigh. RightWeigh is no longer associated with any program, or surgeon for that matter in the area. This why I do not appear on that website anymore. The bariatric surgeons and program from Norton Suburban have affiliated with the Norton Bariatric Center which has been serving the downton Norton Hospital surgeons for the last 3 years. All of the surgeons at the respective hospitals, Norton and Suburban, will continue to perform surgery at the same hospitals. With this growth in the Bariatric Center additional services are able to be added, including bariatric medicine. With this change, the process for my patients will change, with the first step is to attend a seminar. The involvement of the Norton Bariatric Center will also streamline the process with fulltime involvement, where RightWeigh had only operated 3 days a week. Angela, thanks for the comments. However, there have been some changes. RIGHTWEIGH has been terminated by the hospital and no longer represents the surgeons at the hospital. The NORTON BARIATRIC CENTER is taking over all of the functions that RightWeigh had performed. I am excited with this change as we will be able to provide an even more comprehensive service to patients than we could when associated with RightWeigh. I have been performing the Lap-Band for 2 years now and in recent review of my data, it makes up about 50% of my practice and the rest are laparoscopic gastic bypasses. Dr. Shina is getting ready to start doing bands. The first step in the process is to come to a free seminar. This is a little different than the way it was done last year. I'll see you thursday. My office does not collect any fees except insurance copays and any deductable that may be assigned. One program requirement is that you see the nutritionist. By the time of surgery you will need to see the nutritionist twice, at $80.00 each. The first is for the nutritionist evaluation which is also a requirement of the insurance. The second visit with the nutritionist comes 2 to 3 weeks before surgery in the form of a PAC (pre-admission counseling) which is a nutrionist led group class which goes into great detail of what to expect before and after surgery. I perform the Lap-Band in Louisville. Our self-pay price is $14,900.00 for hospital fee, anesthesiologist, surgeons and assistants fee to be performed as outpatient or overnight stay. Adjustments and office visits for at least the first year are included. Most of the adjustments following surgery end up being in the first year. Preop clearance, nutrition eval and psychological eval are not included in that price. Some of these items are frequently covered by insurance even if the insurance doesn't cover the surgery. In the country, so far, there are only about 110 COE by the American Society for Bariatric Surgery or SRC and only 1 in KY in Lexington. The American College of Surgeons also will be providing a COE (Center of Excellence) certification that will be accepted by Medicare, but they have not assigned any so far. At Norton Hospitals in Louisville, the process has been started, just not completed. We have long awaited the Medicare announcement that came 2-21-06, and are happy that Medicare is expanding coverage to include the Lap-Band, but also their guidelines will be more in line with the NIH guidelines. What was a surprise, because it wasn't noted on any previous releases, was that Medicare would require COE certification. An even bigger surprise was that the policy that was released for the first time on 2-21-06 went into effect on 2-21-06. Therefore, all Medicare insured bariatric surgeries in the country will not be covered until each center meets the COE criteria. We are expecting to have this resolved in a few weeks.
