Network Health Forward and VSG?

I keep getting different answers from my insurance company. Has anyone had the VSG with Network Health Forward?

They covered my gastric bypass

Did you get approved? I have Network Health Forward too. I guess I was approved. My surgeon's office has me scheduled for sleeve surgery on November 13th. I just got the call last week. I never received anything saying I was denied.
