Ontario Bariatric Registry Patient Number (for Humber assignees)

My Drs office confirmed my referral had been accepted and I had a patient number. I called TWH and they had no record of me and said my patient number has to start with 041 to be one of theirs.  Mine starts with 003.  I think that might be Humber.  Does anyone who went to Humber have a number that starts with 003?

Your referral goes to the Ontario Bariatric Network who then decides which hospital it goes to.  Perhaps that is the number you were given. I didn't get my hospital card until my orientation and first appointments were assigned at TWH.

So I am asking if anyone who went to Humber had/has an Ontario Bariatric Registry patient number which begins with 003.

Perhaps my reply wasn't clear...unless you yourself have been called or mailed by a specific hospital advising you of orientation, the number you were given was likely from the Bariatric Registry.

No disrespect maybe I wasn't clear.  My Drs office gave me the number I had been assigned from the Bariatric Registry.  The people at Western told me the first 3 digits of the patient number from the Bariatric Registry are the numbers which tell you where you gave been assigned.  I want to know if anyone who was assigned to Humber has the first 3 numbers in their patient number as me.  That's it.

Your doctor can tell you what hospital you were referred to... they would have been told when they put in the bariatric registry..so I would just phone them back and ask them... :)

Thank you, I'm going to phone them back. They just always seem so busy and rushed.  Congrats on your surgery date. June 12 is a lucky day as it's my wedding anniversary!

What BlulzGal was indicating in the post is likely correct.  The number your doctor's office gave you is nothing to do with the hospital patient number but is the registry number given when referral accepted by the Ontario Bariatric Network.  You usually do not receive a number from the facility until you have actually been registered into that hospital, so likely will not receive that until you do.  It was about three weeks before I received anything from the institution for my orientation session and no patient number has been provided as yet.

Referred to Windsor Nov./16, Orientation Jan. 16/17, NP Mar. 6, NUT Mar.21, DR/SW Apr. 19, Approved by Dr., Dietician/SW Jun. 5 Approved, Surgical Class July 24, met the surgeon Dr. Boujani August 3/17, Pre Op Aug. 22, Opti Aug. 29, Surgery Sept. 12/17.

No worries, I understand, I didn't explain myself well enough. This is a question about the 9 digit number assigned to people in the Ontario Bariatric Registry for people who have been or are going to Humber.  I have edited my post topic to better reflect that and I thank and appreciate everyone's patience with me.

My Drs office gave me the 9 digit patient number that they were given from the Ontario Bariatric Registry after the person I spoke to at Western told me the first three digits of that patient number indicates to which centre you have been assigned.  So, all I am asking is if anyone who is assigned to Humber have the same three digits as me as their Ontario Bariatric Registry patient number.  I'm also going to ask my Drs office if they have more info than just the Ontario Bariatric Registry number as was suggested.

Honestly, I have no idea what my Bariatric Registry Number is.
