 Thank you all so much.  Holly's information is really going to help.  I'm trying to help a friend fight her insurance company and she needs a place to start.  :)

 I have given her the information here and it's up to her to contact them.  I'm hoping to keep the fire lit under her.  Her husband works for a self-insured hospital system and the policy states they will cover RNY for the employee but not for any dependents.  Her husband doesn't need and will never need the surgery yet they won't let him transfer the benefit to her.  I think she has a discrimination case here.  It's like saying they would cover the employee if the employee got pregnant but not the employee's dependents.  She deserves to get this procedure and I think she needs to fight for it.
