Many moons ago, someone with a blue rose as an avatar (????) made medical alert cards for a bunch of us.  The front had a color description graphic of the surgery, and the back had the type of surgery, surgical dates, allergies, and surgeon info.  My contact info has changed...and my cards are getting ratty.  Does anyone have this saved as a template?  I'm not the best at cutting, pasting, sizing, etc....    If you do, can you email me a copy?  Thanks,  Mary  C
