Work letter prior to surgery

I was hoping someone in the same field would chime in!! I'm sure it will be exhausting getting back into the swing of things.

I still have some waiting time but I'm so excited now that I just want to have everything ready to go.

I did not need a note for my employer but I only took two weeks off (I have a desk job). From what I understand, the employer can only ask for a note from a medical professional that says you need time off for medical reasons for a certain number of days/weeks. In my workplace (private sector), we are not allowed to ask what the actual medical reason is. Good luck!

He printed it right at computer... it's just a form letter they have there.. :)

I brought my forms with me when I met the surgeon. He filled them out and signed them no problem.
