Insurance Question?????

I am seeking a revision from the VG to a RNY lost from 327 to 145 now back to 208. My VG had no malabsortive only restrictive. I have BC/BS of SC and my BMI is 35 with comorbidities. I have done everything needed now it's just time to submit to insurance. Has anyone seen BC/BS of SC pay??? Thanks for your help.
