Bloodwork results- HELP!
Here are my 2 yr lab work results, the only thing I believe that is not listed on here for some reason is vitamin d- my level which now is 112 and B1 which is 51. 6 months ago my vitamin d level was 36, and B1 was 20. I have been taking the Dry D3 50,000 iu 4 x's a week and Bcomplex 5 x's a week. My PCP said my D and B levels are too high, she said I am at risk for toxicity if I go above the normal range even with being a gastric bypass patient (as she googled the side effects of those levels being too high) She told me to only take 1,000mg of D a day and take the B 3 x's a week. I am getting my glucose checked again (since I was chewing gum when i had bloodowrk done) and also getting my PTH checked again- she said it may be low if my calcium was high?
she also questioned the amount of labs I was getting done, she said she has never seen a post op patient at 2 yrs out get this much bloodwork tested? Don't I have the right to request what ever tests I want and feel necessary? (this is the 2nd PCP I have switched to due to my last Dr. denied my labwork to be ordered) She also said she would prefer my surgeon to review the labwork as well, but my surgeon told me to request all labwork through my PCP. Does all this bloodwork seem overboard? I am so confused and dont know who to listen to, I just want to make sure I am getting the correct labs done to make sure I am healthy.

Your D is not toxic and I would not cut back at all, personally, but definitely not as much as she wants you to. It would quickly drop into deficiency again. You need it above 80. Your iron and protein are great. And your B12. Yes, of course you have the right to request all the tests you want. I get more tests than this every six months! For instance, I didn't see vitamin K, B6, copper or selenium here. Just because she's not used to seeing RNY patients get complete sets of labs, does not mean you should not get them. I would just tell her you know many patients whose surgeons recommend these labs and that you feel more comfortable getting them all done, so please order them. Please note: I AM NOT A DOCTOR. How can you possibly know how your doing without iron, ferritin, vitamin B1, B6, B12, folate, vitamin A, Vitamin D3 = 25 hydroxy, PTH, selenium, copper, zinc, vitamin E, vitamin K? With the few tests that you get, you can't possibly know how your blood levels are and if you are taking the proper dosage of vitamins and if you have any deficiencies. B1, B6 and B12 can cause nerve damage and other neurological problems. vitamin A can cause blindness vitamin D and PTH - are needed to check the status of your bones to prevent osteoporosis iron, ferritin to prevent anemia copper can also cause anemia and neurologist problems. other vitamins have many other symptoms - all necessary. Iget full labs every 3 months and sometime more often and they are all necessary. My doctors order them without my requesting but if I want something extra they will always do it and insurance always pays
I am not sure what vitamins you are taking but most of your labs are good but there are a few problems Your hemoglobin is on the high end of normal which is okay but you don't want it any higher - your iron is also at the top end of normal although your ferritin is okay but not great. What type of iron are you taking and how much? Your B12 and folate are great. A high B12 is fine and yours is ideal. A vitamin D of 112 is kind of high and there is a lot of controversy on this topic. If you were steadily at around 100-110, that might be great but you increased from 36 to 112 and that is a hugh jump. I wouldn't want it to go any higher and even if it goes down a bit, you will be fine. I would take it 3 times per week instead of 4 times per week but the full dosage - dont' cut to 1000mg or your levels will plummet. When I was up at greater than 150, I cut from 3 times per week to once per week of the 50,000 units and my level plummeted to 50 in 2 months.. You want your level to go down slowly so you can adjust it easier. That will also help bring up your PTH. I h ave never seen a PTH so low - ideally we want to be around 30-40, I was at 22 at one point when my vitamin D was high but 22 was okay. The higher your vitamin D, the lower your PTH. What your doctor has backwards is a high calcium causes a high PTH, not low and your calcium is perfect, so it can't be from your calcium - if you cut your vitamin D, your PTH will go up. Your magnesium is really low and for a magnesium blood test to be that low, you are really deficient - our bodies naturally make up for magnesium loss and adjust our blood levels until serious deficiency takes place. You need to get some Magnesium Citrate - you can check with vitalady for a proper dosage but I think it is 250mg with each calcium dose but not sure. It will also help you absorb your calcium better. your liver, kidneys, protein levels are all good. your zinc is on the low side - you might want to get a zinc supplement also, again ask vitalady for the dosage. Do you take vitamin A? Your A level is on the hgih side. I used to be very 'v vitamin A decient and took high dosage dry vitamin A. then all of a sudden my levels went high (120) and now that I am off all vitamin A, I still can't get my levels down. So, if you are taking vitamin A, you should cut your dosage a bit - ideal levels are around 70 or 80 As far as your vitamin B1 - it is fine to have a high vitamin B1 - you just pee out the excess of vitmin B1 and B12 but the problem with B complex is that it has vitamin B6 in it and B6 can get toxic. Not everyone gets there B6 lvels checked and almost everyone that does, finds there levels are too high making B complex not a good idea. It is better to take just vitamin B1 by it self - 100mg per day and that usually keeps you where you want to be or a little high. I was very deficient and I take 300mg per day As far as blood testing goes, yes - you should be able to get the necessary blood tests. I get a full blood panel with all vitamins and minerals every 3 months and have since the beginning - I am over 3 years out. I went 6 months or a year, things would be really bad - I fluctuate a great deal and need monitoring. I am lucky that my endocrinoligst does my blood work, maybe you should find one of them. They are very familari with WLS and what labs need to be done. Many also have NUT's. I don't have to ask but I wan't something extra, I just ask last month I wanted B2 and B3 - she had no problem.
