I'm scared to be put to sleep.
During preop, I was terrified that I wouldn't wake up. My bedside nurse was an older woman, probably near 70's, and she was wonderful! I wish I could remember her name, or find out who she is, so that I could send her some flowers or at least a card to thank her. She was an angel...very kind, soft spoken, gently touching my cheek as she reassured me that she wouldn't let anything happen to me. I've not had anyone be so kind as this woman was to me.
Hopefully, you will have someone as sweet as my nurse.
When I expressed my fears to my surgeon, prior to the surgery, he told me that almost no one dies on the table. It's the complications afterwards that take them...lol THAT made me feel really comfortable. But, seriously, you'll be fine. We are all here after being put to sleep! :)
Versed is a wonderful thing - make sure the anaesthesiologist gives it to you once he/she have had their chat - it will take all your cares away!!!!
Meanwhile, try to think of how many people are anaesthetised every day without incident - you have had all those pre-op tests to make sure you are in good condition for the surgery so you really have nothing to worry about.

I asked for something in the I'VE to calm me and it did. I was awake even transferring to the operating table but the calming member helped me to feel like I was part of the team. I asked to be told before they put me out which they did. After I woke up, I always made sure the pain was addressed to the max. Which helped me to go with the flow. The first two weeks are hard work, though,maombe prepared for that and get some support for that time. Keep us posted!
I used this guided imagery CD and it helped soooooooo much. You can download it for $13, totally worth it. You can listen before and during surgery (if they let you which they usually do). I have major medication establishment/doctor/hospital issues and this just made me feel soooooo much better. I just loaned mine to someone or I would send it to you. It will be okay.
