2/17/06:  Met with PCP to discuss Weight Loss Surgery

2/23/06:  E-mailed PCP to check on status of referral to Bariatric Dept

3/9/06:   PCP in Vacaville submitted referral to Bariatric Dept

3/15/06:  Received phone call from Richmond Bariatric Dept to schedule Psych Eval & Nutrition Consult (my PCP did not do this before she sent in the referral) and was told that I was accepted into the program.  Later that day, I received an e-mail from PCP stating that I was accepted into the program.

3/28/06:  Had Psych Eval & Nutrition Consult in Richmond.

4/28/06: Received "THE PHONE CALL" to schedule Orientation

5/9/06:   Attended Orientation

5/21/06:  Started recommended 1200 Calorie Plan

6/6/06:  Attended mandatory Nutrition Class

7/5/06:  Attended Surgery Consult Appointment with Dr. Baggs

7/5/06:  Ordered vitamins from Bariatric Advantage.

7/11/06: Weight-check at Richmond Bariatric Dept.  Made Pre-Op Goal Weight!! Case Manager was able to squeeze me in between appointments.  Surgery date scheduled for 8/7/06.  Had EKG and blood work done.

7/13/06: Started taking post-op vitamins.
 
7/19/06: Attended Pre-Op ROUX-EN-Y Teach Class in Richmond

7/20/06: Attended Bariatrics Pre-Op Class in Richmond

7/27/06: Attended final 3 appointments (Dr. Baags, Anesthesiologist and Physical)

8/7/06:  Surgery day!  Everything went as planned.  No complicatons.  I was able to go home the following day. I am now off from work for 3 weeks.

8/28/06:  Back at work, everything going well.

9/5/06:  Had 4 week check-up with surgeon.  All going well.  Weight loss right on track (according to surgeon).
 

10/15/06:  Reached 191lbs = BMI is now 29.9, no longer Obese
 

11/2/06:  3 month post-op appt.  Labs came back good.  Moved to Stage 4.
