OK MY INSURANCE IS WELLCARE OF GEORGIA, CAN ANYONE TELL ME WHEN THEY ASK FOR PROOF OF THE SIX MONTHS SUPERVISED DIET DO THEY WANT THE DOCTOR'S NOTES OR DO THEY WANT A SUMMERY OF THE DIET FOR THE LAST SIXS MONTHS. I'M ASKING BECAUSE THE SURGEON OFFICE GAVE ME THESE PAPER'S THAT LIST EACH MONTH 1THRU 6 AND MY DOCTOR CAN JUST WRITE IN WEIGHT, DIET AND EXERCISE exct.... WILL THIS BE ENOUGHT FOR THE INSURANCE COMPANY FOR ME TO GET APPROVED? ANY INPUT IS NEEDED AND THANKS IN ADVANCE.

Can anyone please tell me if i've been seeing a LCSW for my psych eval will my insurace wellcare except a cleareace letter from her so I can get wls or do I need a letter from a psycologist.The Surgeon office has a psycologist on staff but She charge's $300 and I don't have $300 so I called wellcare and asked them to find me a psycologist that they cover so I can get cleared for wls and I made an appointment with who they referd me to.so I went to 6 session and she wrote the letter and she say's to me do you think your surgeon's office will take this cause i'm not a psycologist i'm a lcsw and I was like I thought she was a psycologist and I told her why I needed this clearence and now i'm so hurt. I been praying that my surgron's office say's that my insurence accepts the letter she wrote.It covers everything it's just that she's not a psycologist she a lcsw.
