Jan 13/09- faxed in ammendment to change surgery type from MGB (no longer covered by OHIP) to DS.
Jan 20/09- denied for DS
Feb 02/09- faxed in ammendment to change surgery type from DS to VSG
Fen 05/09- APPROVED!
