On the Road Again ...

Just wanted to give an update of sorts.

I had my scope done yesterday. It was really easy & I kind of treated it as a dry run for surgery since anesthesia seems to cause the most problems for people. I handled the propofyl (?) very easily & the whole thing was a lot less scary then when I was knocked out using gas as a kid. My procedure was scheduled for 9:30 am & I would say I was in lala land by 9:45 then woke up in recovery at 10 on the dot. Dr. Hoddinott found signs of GERD but that was totally expected. He did 2 biopsies of my stomach & said everything looked good.

Madaline submitted my info to insurance on Aug 3rd so now we wait to hear back from them so I can get my surgery scheduled. I am going to wait (even if it kills me) until Monday to see if she has heard anything back. I REALLY need to have surgery next week so I can have the drain removed prior to starting class on the 28th. It won't be the end of the world if I have to go w/a drain but seriously, who wants a tube hanging out of your gut when you are walkin the halls with a bunch of fresh faced freshmen?

A nice lil victory I won w/my daughter today is that she is FINALLY willing to eat those lil pouches of fruit puree. I have tried a couple different pouches about every 6 mths & today she finally took to them. She thinks they are a treat & I gave it to her when she started asking for ice cream (I don't let her have the junk but living with grandma it happens). I am really trying to keep her from being an overweight kid so score 1 for momma!

Whoops! My Bad ...

I forgot to update my experience with the psych eval & the nutritional class. Oppps ! 
I had both appointments on June 26th. That was a crazy day, I would really recommend spacing those bad boys out a lil more. My psych eval was in Orlando, then I had to go to Ocala for the nutritional class. Bleh!

Overall the psych eval wasn't too bad. I really liked the counselor I finally ended up meeting with. One thing that would have helped is if the office actually DID pre op evals! When I called to make the appt; that was my first question & I was told, "Yea, no problem. We will take care of that for you and you will get a letter to give to your surgeon." Aces! Sign me up.

Well google maps gave me half assed directions & it ended up taking me 2 hrs (during Orlando's morning rush hour) to make it to the office. Then I get called back, sit down with the first counselor & when I tell her why I'm there she just sits there stuck on stupid & tells me, "I don't do those." Uhm ... 'Xcuse me? Yea ... all she would/could do is a general psych assessment & then she refused to even do that b/c of liability. I call bs on that point but whatever. There was a second counselor there w/an opening an hr later & she agreed to see me & do a general assessment. The office staff really felt bad & ended up doing right by me.

Since it wasn't a traditional pre op eval; I didn't have the ginormous questionaire. I basically led the whole session with her asking a few probing questions here & there to draw out more detail. We talked about the anatomical, physical & dietary changes that would take place, my weight loss history, my support group & why I felt this time would be different. The weirdest part were the questions that go along with the general assessment. Things like; "Are you suicidal, have you had homicidal thoughts (Uh hello? I just drove 2 hrs in rush hour just for yall to tell me; opps nope we can't help you. Yea... I've had a few lol), do you have a history of eating disorders, addictions & what meds I was currently on." Not a big deal. My appt was on a Tues & she had my authorization letter written & submitted to my surgeon by Friday.

So after the psych appt; I dashed to Ocala so I could make it to the 3:30 nutritional class.
Turns out ... I was the only one to show up. Hmm ok. I was looking forward to meeting others that were going through this process with me but it turns out everyone was there the previous week. Ok. Fine. Be that way lol.

My dietician is soooo sweet & he is from a small town too so we kinda clicked. For anyone going through Dr. Hoddinott's office; his name is Daniel & I would recommend him to anyone. We sat down with the "plan" & went over every page in a lot of detail. He was great to answer questions and didn't make me feel like an idiot.

Here's a general run down on Dr. H's diet plan:
The first 4 wks consists of 3 meals (1c over 60 mins) & 3 snacks (4oz protein shake over 30 mins). Personally, I don't think it will fit & we will probably be on much smaller portions but I think the point is we are supposed to take the entire time frame to eat/drink ... so basically as much as you can get in at 30 or 60 mins. 

Wks 1-2: Full Liquids
Wks 3-4: Everything from Full Liquids + Pureed

Oh & Dr. H's rule on water is to drink right up to the meal & then wait 30 mins after your last bite to start drinking again.

So yea ... That was my big take away from the day. All in all not bad.

I just talked to Madaline today & my coverage was screwed up so we are going to hit the ground running on Aug 1st & hopefully I will have my surgery by the end of August. That leaves me NO time before classes start but it's only 2 classes a week & I only really have to have my laptop w/me. ::Sigh:: I can't wait for my spot on the loser's bench to open up. Soon though.

It's All Happening

So I finally had my consult on Friday (June 15th). It was a super long wait & the waiting room was full. The plus side of the wait is that the staff was really nice, explained they were running about 45 mins behind & didn't call patients out of order. (I hate when you are waiting forever & see someone that walked in after you gets to go back first ... just irks me lol) Anywho ... 

While I was waiting I got the chance to talk to 3 of Dr. H's post ops, 2 yrs, 1 yr (a revision from another surgeon) & 6 mths out - they all had nothing but the best things to say about him. When I got called back I was weighed & put in a room (I'm down 13 lbs since April Woot!). Dr. H was really nice & broke everything down Barney style. He said I'm goin to be a rockstar & my surgery will be a treat for him since I am technically much smaller than a lot of his patients.

After Dr. H went over everything I saw his assistant & she is great. She said with my insurance I would need a 6 mth supervised diet, a psych eval & per hospital requirements a nutritional class with the dietician before they could submit my info for approval. Well I have been seeing my PCP for a yr & on each of my visit forms she notes that I am overweight & we discussed a low fat / low cholesterol diet. Madalyn (? I'm horrible with names) said that was perfect & she just needed to get those records from my PCP. 

As of today I am scheduled for my psych eval & the nutritional class for Tues June 26. So I am hoping that they are able to submit my info by Friday.  I am really hoping that everything works out and that I can have my surgery mid July.

I can't believe that is all that they required. I was really prepared for a long drawn out process but everything seems to be chuggin right along. I will post again on Tuesday so anyone following will know what to expect from those appointments.

Ok .. This week has been brutal. B-R-U-T-A-L ! 

On Saturday I started Atkins again. (shoot me now) I went into this really positive that I will be getting my surgery soon and I thought if I could go ahead and get the carb withdrawl out of my system that it would put me on more stable ground for post-op. I tend to get really depressed, migraines & extremely tired to first 2 wks on Atkins & didn't want to go through that post-op and have a negative outlook on my new life.

So here I am ... day 5 of my carb withdraw hell (You don't even WANT to know what I would do for a klondike bar at this point). I got a call from my surgeon's office yesterday, I returned the call, left a message and didn't hear back from them until I called AGAIN today. (My consultation was scheduled for Friday btw).

The woman gets all huffy and tells me "Oh yeah ... We cancelled your appointment because your doctor didn't send us the referral information". Uh excuse me? I couldn't make an appointment without the referral and the guy had the referral in his hand when I made this appointment ... a MONTH ago!

Then she goes on to tell me there was other info they needed and my dr's office should be familiar with the process b/c they have 8 other patients going to the same surgeon who have the same insurance that I do. ... Ok ... Let me call my PCP's referral cooridinator and find out what's going on.

Little backstory: I first talked to my PCP about RNY on March 9th. That's when she submitted the referral to be sent for authorization to my insurance (since the surgeon is out of network - no big deal - she also told me that the woman in the office would sit on the referral for 3 wks then I should hear from them.) 3 weeks pass, I hadn't heard anything so I called my insurance company to get an ETA on the authorization. The rep says, "Prior Auth? We don't have one on file for you ... The dr never sent us anything requesting you see a bariatric surgeon" .... oooooook ... Call the PCP and talk to the referral coordinator ... She tells me, "Oh well I couldn't find a surgeon to recommend you to .. there isn't anyone listed on the website and I called the ins comp but they couldn't tell me b/c they don't have a surgeon in network." Seriously? So I let her know that 8 other patients, in the same office/with the same insurance, are going to Dr. H ... He takes my insurance. So fine after telling her how to do her job the referral was sent and approved within 24 hrs. I get the approval, the PCP faxes that to the surgeon, the surgeon verifies the referral and I make my appt for a consultation 1 month out b/c that is the first available appointment.

Now here I am ... 2 days before my consult and finally starting this process to have my surgery and they drop this on me. I got pissed ... I got upset and I cried. Now I'm done crying and I am really friggin pissed.

I called my PCP's office & spoke to another referral coordinator and she tells me that the woman working on my case (the same ditz I spoke to a month ago) was on vacation and so she didn't know what was going on with my case but also didn't see any notes where the woman has submitted the additional paperwork that is required for an out of network referral. But since it wasn't her case; the woman I spoke to wouldn't work on it for me.

So I am stuck until in the morning. Please believe me when I say ... Everyone in that office will know my name by the time I am done. My referral coordinator will be getting a call at 10 am and if I have to hold her hand through the whole process ... my info WILL be submitted to the insurance and sent to the surgeon by the end of the day. This consult has been 4 yrs in the making and I'm not letting some incompetent twit screw this up for me.

Here's my question ... Why am I just learning there is a problem 2 days prior to my appointment? The surgeon's office has been looking for this paperwork for a month ... Shouldn't they have given me a heads up a little bit sooner so I could make sure everyone was doing their job? I'm filing a complaint with my PCP & I want someone else on my case. I can't let this woman be in control of my health.

So yea ... That's where I stand in the process ... A big ass yield sign flashing ...

Oh where to start?

Welp let's start here ... I am 28, overweight and a single mom.

I have been overweight my whole life and want to keep that lifestyle away from my daughter.
So I have set out to change things.

I have been interested in WLS for a few years now but kept convincing myself that I wasn't "that big" and that I could just buckle down and lose the weight.

I did really good on Atkins a couple years ago and lost 50 lbs. Then I got pregnant and gained 35 (granted a lot was water weight due to pg induced high bp). Within 2 wks I have lost ALL of my baby weight. (Woot!) Now my daughter is 2 and I am pushing my pregnancy weight ... eek! 

So I have gotten the ball rollling to meet with a surgeon. I have attended a seminar to learn about my option. I had gone in thinking I wanted lap-band but the results are less than stellar. So now I am pushing the RNY. I have found an excellen surgeon, who is close to home & has a great reputation.

I had to get pre-approval on my referral from the PCP before I could even schedule a consultation; so that has added a lot of time to my wait. My PCP submitted the referral on March 9 - it was approved on April 12th. After I had to play phone tag between the PCP and Ins Comp. So now that it was approved and sent to the surgeon I called today to schedule my consultation. The first appt is May 11.

I have been checking out his patients comments on the boards and he seems to get his patients in and out of the process pretty quickly. Most that I have seen have had their initial consultation and surgery 4-6 wks apart.

So here's to hoping right?

I know for a fact he has 8 other patients from my dr's office that also have the same insurance. So I would assume it would be a very streamline process. The only thing that is concerning me right now is my dr mentioned my ins requires 6 mths of diet plans. I have been going to this dr for a yr & have been almost every mth for follow ups. On my visit breakdown sheet it always says to follow a low cholesteral diet ... So not sure if that counts. But my dr also said they now have a "medical neccessity" letter that will bypass the 6 mth diet plan.

The only reason I'm in a time crunch is that I am also a full time student; working toward my Paralegal degree. It is uber demanding and I am out of class from May - Aug and would loooooove to be able to have the surgery during my break from class.

Time will tell.
