I need to ask anybody about the psyh visit, my surgeon recommend 3 different ones and said that my insurance does not pay for the report and that it is 150.00 .  Is there anyone here from Youngstown or Warren, Ohio who has went to a appt. where the doctor didn't charge that much to write the report?
