Finally mailed out questionnaire of 20-something pages filled out with my life history...
Was in denial and probably a little embarrased that I couldn't take control of my life, so I kept finding excuses that I did not have the time or didn't have all the answers on my medical history... So I kept putting it off, well no more!
Will be contacted if eligible for WLS... And the waiting game begins... :-)
