I wish to appeal your decision for denial concerning gastric bypass surgery to be done on March 20,2006 by Dr. Jonathan Wong at Long Island College Hospital in Brooklyn, NY.
My name is Stephanie Ryan-Jagroop and I suffer from a DISEASE called morbid obesity. I am 26 years old. I have been overweight ALL of my life! I don't want to live my life this way anymore.
I have been doing research on gastric bypass for the past 2 years now, and I know this is the best surgery for me. I consider this surgery to be my last resort. I know all about the do's and dont's before and after surgery. I know the risks and benefits this surgery has to offer. After the surgery I will adhere to the diet plan regimen. I know what will happen to my body If I do not stick to the plan assigned to me such as dumping syndrome, protein and vitamin deficiency and others. I have tried all other diets and failed; such as-
diet programs:
low fat
low carb
atkins
slim fast
weight watchers-also attended meetings
excercise is included in all weight loss atempts
diet pills:
xenical
meridia
hydroxycut
metabolife
various gnc powders and shakes
Any weight that I have lost in the past was gained back with extra pounds and just keeps on....
I currently stand at 5"3 and weigh 264 pounds with a bmi of 46.8= extremely morbidly obese
As time goes on I just gain more and more weight and my body can't handle this.
I currently suffer from-
-lower back pain
-physical activity is limited
-shortness of breath from minor exertion
-heartburn
-acid reflux(currently taking prevacid)
-pain in my feet and legs(currently have heel spurs)
-infertility
-irregular menstrual cycle
In addition-
-I don't want to be seen in public
-embarrased to meet people
-feel like i am a failure
-poor self-esteem
-lacking in self confidence
-embarrased to travel
-held back from doing things
Previously, my surgeon submitted a request for pre- authorization for the gastric bypass surgery, In the files sent were

I am asking that my case be reviewed as soon as possible. I understand that as of now I have no co-morbidities, however if this weight problem of mine continues I will suffer from serious health risks, resulting in a shorter life span.
Please keep in mind that-
I have tried to lose weight many times unsuccessfully with nonsurgical measures
I am determined to lose weight to improve my health and quality of life
I am aware how my life will change after the operation
I am aware of the potential for serious complications, occasional failures, and dietary restrictions
I am well informed about the surgical procedures and the effects of treatment
I am committed to lifelong medical follow-ups
I currently joined a support group online at obesityhelps.com, I personally know 5 people who have had gastric bypass, and I also have great support from my husband, family, friends and co-workers.
