Dr. Miles seems very down-to-earth, and is easy to talk to. He answered my questions fully, and gave explanations in terms that weren't over my head, yet he didn't talk down to me. He fully explained the risks of surgery, but he was very upbeat, and confident, which was very reassuring.

Hi, my name is Tami, and I'm an obesity survivor!  A little over 18 months ago, I was 273 lbs., and wore a size 26.  I had high blood pressure, plantar fasciitis, and suffered from aching joints.  I had low energy, and avoided shopping and social activities.  Then, in July '06, I attended a seminar on bariatric surgery, and I knew it was time to take charge of my life and health.  

I had lap band surgery by Dr. Les Miles on September 5, 2006.  Since then, I've gone off 3 prescription medicines, wear a size 6/8, my heel spurs are gone, my hips and knees no longer ache, and I've lost 124 lbs.  Before losing weight, I avoided physical activity.  After surgery, I joined a gym and started exercising.  I now feel strong, energetic, and physically fit.  A cloud of apathy has lifted from my life.  I'm much more self-confident, and ready to take on new challenges.

I am so glad that I made the choice to have weight loss surgery.  The Lap Band gives me hope and confidence that I'll keep the weight off long-term.  Life is too short not to live it!

Well, it looks like I've successfully kicked the "Obesity Help" habit.  Whether that's a good thing or not, remains to be seen.  But, I have to admit that I sometimes used the site as a "procrastination tool".  If I really had other things to do, but didn't feel like getting started right away, I'd tell myself, "I'll just pop onto OH for a minute or two", which often ended up being an hour (or two!)  

I've been busy with the new job as Spinning Instructor.  I only have a couple of classes a week (or more if I sub), but I'm enjoying listening to and downloading new music, reading about Spinning and fitness, and getting involved in the online Spinning community.  I think of myself as an apprentice in the fitness industry, learning as I go, and I have a LOT to learn.   But, the classes are a lot of fun.  I already have a great group of "regulars", and that means a lot to me.  

I attended our local lap band support group meeting last weekend, and was told that they had loaded videos from the Bariatric Fashion Show on the St. Vincent's East web site.  So, at the trade off of my anonymity online, I'd like to share it with ya'll.  There are many inspiring stories within our group.  What really amazes me is that I made it through the fashion show with makeup intact.  I kept getting teary-eyed listening to everyone else's testimonies!

The bariatric program at St. Vincent's East, in Birmingham, AL, is awesome.  Participating in the fashion show was one of the highlights of my year, and the video is something that I will treasure forever.

I'm baaack!  The conference was awesome.  I participated in some incredible rides, and learned so much in the lectures and workshops.  I also met some wonderful people, who I hope to keep in touch with.  The filming of the commercial was interesting.  I hated the whole interview process, but the makeup and hair, "costume" changes, still photos and the "action" filming (of me Spinning) was kinda fun.  I have no idea if my stuff will actually get used, because they had several people who were filming, and I'm sure they'll use whatever (whoever) best represents their product.  I was told that they expect the commercial to be ready to air by September.

Just a quick update.  Things are going well.  My weight is stable at around 150 (give or take a pound or two).  I got a slight unfill this week, since I've been on the tight side since the port replacement surgery.  I'm now back to eating still-warm food again, hooray!  

I'm leaving tomorrow to attend the World Spinning and Sport Conference in Miami, FL, and to be filmed for an infomercial on Spinning.  Who knows whether or not I'll be included in the final production, but I'm excited about getting the free trip and being able to attend the conference.  

I'm in the process of becoming a fitness instructor, and have accepted a job teaching Spinning a couple of days a week.  I just love saying,  "I'm a fitness instructor."  I certainly never thought 21 months ago that I'd ever say those words, and IT FEELS GOOD!

Obesity Linked to Fat Gain in Muscles on 10/14/2005

FRIDAY, Oct. 14 (HealthDay News) -- The skeletal muscle of severely obese people is specially programmed to amass fat, researchers report.

The finding suggests the muscle has a metabolic "memory" of obesity, and it may explain why obese people have such a hard time losing weight, even when they cut calories.

The study in the October issue of Cell Metabolism also suggests that exercise may be able to override this programming and improve the ability of the obese to shed unwanted pounds.

Researchers found that the fat-building enzyme stearoyl-CoA desaturase-1 (SCD-1) is three times more abundant in the muscle of obese people than the muscle of lean people. This finding provides insight into an important link between obesity, diabetes and abnormal fat buildup in muscle, the researchers said.

"Obesity and type 2 diabetes are strongly associated with abnormal lipid metabolism and the accumulation of fat droplets in muscle, but the underlying cause of these perturbations have been unknown," study author Deborah Muoio, of the Sarah W. Stedman Nutrition and Metabolism Center at Duke University Medical Center, said in a prepared statement.

"We've now shown that SCD-1 is at least a very important contributor to changes in lipid handling within muscle and the progression of obesity," she said.

She and her colleagues plan to examine whether increased physical activity can reverse levels of SCD-1 or help neutralize its effects. It's known that exercise produces changes in muscle metabolism.

I'm so excited!  I got the call, and I'm going to Miami to participate in the National Spinning Conference at the beginning of June.  Someone sent me a PM, asking what Spinning is, and here was my response:

Spinning is indoor cycling.  It's usually taught in a group setting, with an instructor leading the class.  They have really good, energetic music going, and special lighting.  It gives you a group exercise experience without having to learn any complicated movements.  You don't even need to know how to ride a bike!  With a few easy hand and body positions, and changes in resistance and pedal speed, they put together various riding "profiles" that keep it fun and interesting.  And, as you develop your fitness level, it can be one of the most intense and *effective* cardio workouts possible.  Best of all for those of us who've suffered obesity, Spinning is low-impact, so it's easy on the joints.  The most difficult thing for some people is getting used to the saddle (or seat), but there are gel-padded bike shorts or seat covers than can help.  And, you just get used to it, after the first few classes.

Lifestyle Changes

Weight loss success is not about whether you choose gastric bypass, the band, DS, or even if you choose to try one more time to lose weight without surgery.  It's about making lifestyle changes.  If you want to be healthy and thinner long-term, then you simply cannot continue living the life that got you fat in the first place.  You cannot return to those old habits, even after reaching goal.  Diets are by nature temporary.  They are a short-term change in eating, and although many of us have been successful in losing weight by dieting, how long did it last?  I've been at goal twice before with dieting, and both times I was at goal for maybe 5", LOL.  

I've now been in my goal range for about a year, and that's a huge milestone for me.  I always said to myself that if I could make it a year, then maybe, *MAYBE*, this time it would be for good.  But, I can't honestly say that it's the band that has gotten me to goal, and kept me here for a year.  The band *helped* me make lifestyle changes.  The band helped me with portion control and hunger management.  But, I had to learn to eat healthy, whole foods, to do a LOT more cooking, to really be consistent with drinking my water, and to exercise consistently and progressively.  

Please don't count on the band to get you to goal.  Those who do, tend to go for fill after fill, hoping that the band will stop them from eating.  By doing that, you're putting yourself on a choke-chain diet that does absolutely nothing to help you learn better habits that will keep the weight off in years to come.  Understand that lifestyle changes take time, and you can't speed the journey by yelling, "Are we there yet?"  But, by taking a step at a time in the right direction, you'll eventually get where you're going.

I just got off the phone with a rep for a production company that's doing an infomercial for Spinning.  I had sent in a testimony about Spinning, and if I'm selected, I get a free trip to Miami to participate in the national Spinning conference and possibly be filmed for the infomercial.  The downside?  It's the weekend that I'm scheduled for plastic surgery.  I asked her if they could let me know within the next couple of days, and she said she would.  I'll have to cancel surgery by Friday, if I want to get my deposit back!  I have mixed feelings about this.  I really want to go to the conference, though, and if I get picked, then I'll just have to assume it was meant to be!  I'll then have to reschedule plastics for sometime this fall, because I don't want to go through recovery in the hottest part of the southern summer!

Success Breeds Success

A response I made to a post on the lap band forum:

"what makes the commitment to these things different now with the band than they were before the band?  Why are you succeeding with the band and you couldn't succeed without the band?"

I think that success breeds success.  That's why I strongly encourage new bandsters to do all they can to get started on the right foot right out of surgery, by eating healthier foods and following the "band rules", even before they have restriction.  Maintaining *Momentum* is so important!  When you see results on the scale and in your clothes, it makes it easier to stay the course.  And *gradually* you simply develop better habits.  

So, if you're struggling, what I recommend is picking one (or two) areas where you know you need to improve, and focus on that area consistently until you have it firmly established.  So, if you know you haven't really been drinking all your water, start each day by filling a jug with 64 oz., and commiting to drinking it all before dinner (so you're not running to the toilet all night!).  Or, if you've been grazing, commit to no more than two 100 cal. snacks per day.  Or, if you're not sure if your portion sizes are on track, weigh and measure daily for a couple of weeks, and log it into SparkPeople.com.  Whatever you choose, commit that you WILL NOT go two days in a row without fulfilling your commitment.  After 2-3 weeks, you'll have that one under your belt, you'll feel better about your ability to change, and you'll be ready to take on a new challenge.

I got an email this week from Spinning.com saying that they were looking for testimonials about Spinning success stories, for possible filming by a production company doing commericals for new "at home" Spinning bikes.  Those selected get a free trip to Miami, hotel accommodations, and entrance to the national Spinning conference.  I thought I'd share with ya'll my testimony:

My name is Tami, and I’m an obesity survivor!   

A little over 18 months ago, I was 273 lbs., and a size 26W.  I had high blood pressure and suffered from aching joints and heel spurs.  I had low energy, and avoided shopping and social activities.  At age 47, I decided that it was time to take charge of my life and health. To help facilitate weight loss, I had lap band surgery in September '06, which helped with portion control and hunger management.  Within a few months, I had lost over 70 lbs., and my doctor told me that although I'd developed much better eating habits, if I wanted to get all the way to goal, exercise would be essential. 

I joined a gym, and started slowly, but consistently.  One day, I was on the elliptical, chatting with the person next to me about the "elite" athletes who were heading to the Spinning room, and saying how I wished I was fit enough to join them.  A guy nearby overhead me, and stopped by to talk to me about Spinning.  He had recently completed Spinning Instructor Orientation, and told me that Spinning is a great low-impact exercise for anyone, at every level of fitness, and that each individual can customize their workout by simply adjusting their resistance and cadence.  A few weeks later, my gym offered an "Intro to Spinning" course, and I decided to give it a shot.  I've been addicted ever since!  I'm very lucky to have a Master Spinning Instructor, Lisa Coen, who leads classes at my gym, and she has not only instilled in me a love of Spinning, she's taught me a lot about my metabolism and how to maximize fat burning as well. 

Since starting Spinning in February '07, I've lost an additional 54 lbs., for a total weight loss of 124 lbs.  Even better, I’ve gone off of 3 prescription medicines, dropped to a size 6/8, my heel spurs are gone, and my hips and knees no longer ache.    I now feel strong, energetic, and physically fit.  I recently came in second place in a fitness challenge at my gym!  A cloud of apathy has lifted from my life.  I’m much more self-confident, and ready to take on new challenges.  I plan to take the Spinning Instructor Orientation myself in a few weeks, and hope to share my love of Spinning with other people who want to get fit.

I feel so very lucky to have had the opportunity to recover from obesity and regain control of my health and fitness.  Life is too short not to live it!

This *Ain't* a Diet!

An exerpt from a post of mine on the Lap Band Forum:

When I was in weight loss mode, I was pretty strict with following the band rules and avoiding *most* simple carbs and liquid calories, but I never completely avoided them.  From my previous experiences, I'd have to say that would be *my* "road to perdition", because invariably at some point I would give in, have something that was on my "no-no" list, and then feel like a failure and that would be the beginning of the end.  

This time, I didn't want to be on a "diet" that I would eventually go off of.  So, I ate pretty normally, but way *better* than I did pre-band.  I eat healthy, whole foods...mainly lean proteins, veggies, whole grains, some fruits, and "good" fats.  But, even while losing, I'd have some wine on the weekends, and once a month or so I'd share a dessert while I was out to dinner.  

If I was out running errands, I had {{{shock}}} FAST FOOD.  But, my choices were usually Wendy's Chili, Taco Bell taco salad without sour cream, or Chick fil-a chicken nuggets 4-pack with a fruit cup.  You CAN eat fast food without going off the deep end!  

But, every now and then, I'd start slipping into habits that worried me.  When that happened, I went back to journalling my food (SparkPeople.com is great), or I'd go on a "diet" just for a few days to transition back to better quality foods.

The Yo-Yo Syndrome

I've lost and gained *several* people over the years.  I've been all the way to goal a couple of times, but didn't stay there for more than five minutes.  I've now been in my goal range for about a year, and have my fingers crossed that *this time* it's gonna stick.  Here's my theory on the "yo-yo syndrome":  every time you lose weight, you are restricting calories, and you lose *both* muscle and fat.  Unless you are actively strength training during the weight loss phase, you can't help but lose muscle, too.  The result is that your metabolism slows as you lose weight.  Then, you get to nirvana -- goal! ... and you gradually revert to old eating habits, meaning more calories consumed.  You gain weight....and here's the kicker...now you have even a higher percentage of fat than you had before losing weight!  It's *easier* to gain, and harder to lose the next time.  (And, yes, this *can* happen even with the band).

That's how I've lived my entire adult life.  But, there's good news.  You *can* change your odds of reaching goal and staying there with strength training, which will help improve your metabolism and prevent plateaus and regain.

149...149...149...149...149!!!  I have reached Nirvana!  The Holy Grail!  Utopia!  Shangri-la!  The Pot o' Gold!  I have not been below 150 lbs. in over 20 years!

Now, let's see if I can get 'er to stick!

I am officially scheduled for plastic surgery once again, this time for Friday, May 30.  I decided to drop the thigh lift, since that was the part that was the most intimidating to me.  I'm getting a breast lift with augmentation and a lower body lift with auto augmentation.  He'll only do muscle repair of the abdomen if necessary.  Then, I may decide to do the thigh lift later on in the year.

I'm so confused!  I went for a second consultation with Dr. Core about plastic surgery.  I know he's a well-respected plastic surgeon, with something like 30 years of experience.  His practice focuses primarily on face lifts, breast work and abdominoplasty.  But, he has limited experience with lower body lifts/thigh lifts, and only had 3-4 sets of Before and After pics for lower body work.    

The recovery from the port replacement is going well.  I went to the gym earlier today, but only did 30" on the elliptical, at a "granny" pace.  Plus, I walked the dogs twice.

Today's weight:  151, my lowest weight yet!  I haven't been trying to lose weight, but I have to admit that it would be nice to see one hundred forty-something. 

I seem to be healing well from the port replacement, but still sore.  Can't even walk the dogs comfortably, let alone go to the gym.  I'll take it easy again today, and hope to get back to normal (but light) activities tomorrow.  

I have more restriction than I've had in months.  I'm not sure if Dr. Miles added more fill than I had previously, or if it's from swelling.  I've had to really slow down and re-learn how to "eat like a bandster".  

This morning, Alyssa (my 17-year old) told me that a couple of friends of hers were looking at a home video from a couple of years ago, and I was in it.  They said it didn't even look like me, or that it looked like it was "Ms. Tami in a fat suit."  Alyssa said, "Mom, it seems like you've been skinny forever, and it's hard to remember you from before."  Funny how quickly they forget!  It's only been 19 months since surgery!

I had my port replaced yesterday with the low-profile port.  When I asked Dr. Miles about topping off the fill level, he said he'd prefer to wait a few weeks, and add it then, if I need it.  And, it's a good thing, because I swelled up last night and couldn't even get down water, which meant I also couldn't get down any pain meds.  This morning, I started off with warm cocoa to prime the stoma, and I still feel tight, but can get down liquids.  

When the anesthesiologist came in to talk to me yesterday, the nurse asked me about my lapband experience, and how much weight I'd lost.  I told her "120 lbs., no problems whatsoever".  A little while later, I heard the anesthesiologist talking to someone out in the hall, "She lost 120 lbs!  The band really does work!"  Hehehe

My local lap band support group is meeting today, and the topic is exercise.  I'd love to go, since exercise is my passion, and something I love to discuss, but I probably should skip it.

I'm getting my port replaced tomorrow with the low-profile port.  I'll have to wait six weeks after the port is replaced (due to the off-chance of infection) before any plastics.  I'm not 100% sure whether I'll get *any* work done, because I kinda go back and forth on the issue, but at least having the port done in advance means I don't have to juggle getting that done with other work.  

Lisa told me they'll "top off" my fill level, too, since it's been almost a year since my last fill, so who knows?  I might even lose a few more pounds in the next six weeks!

I just made the most awesome dinner, with no recipe.  I'm gonna post it here, not so much for people to try the same thing themselves, but for people to be willing to *experiment* with what's on hand.  You never know, you might create a winner!

My inspiration tonight was 3/4 jar of Classico "Spicy Tomato and Basil" spaghetti sauce in my fridge.  I also had a couple of chicken sausages that needed to be cooked, and some mushrooms that were on the verge of spoiling.  Here's what I created:

Ingredients:

Olive oil
1/2 lb. mushrooms, sliced
1 sweet onion, halved and sliced
1/2 red pepper, sliced
1/2 yellow pepper, sliced
4 cloves garlic, minced
3/4 jar Classico spaghetti sauce (I used Spicy Tomato & Basil)
2 Aidell's chicken sausages (I used Sundried Tomato)
8-10 slices pepperoni (microwave 30 sec. & pat off oil)
1/2 lb. shrimp, cooked, peeled & deveined
1/2 box Heartland whole grain rotini pasta, cooked
1/4 cup shredded Italian blend cheese

Bring non-stick skillet to medium high heat.  Add a tsp. or two of olive oil.  Add mushrooms, and saute 3"-5" until lightly browned and softened.  Place on plate and set aside.  In same skillet, sautee peppers, onions, and garlic in small amount of olive oil for 15" or so, until softened and lightly browned.   Place on plate with mushrooms and set aside.  Sautee sausages for 2"-3", add shrimp and pepperoni, and heat for a minute or two until hot.  Add sauce, sauteed veggies, and cooked rotini pasta.  Heat, and top with shredded Italian blend cheese before serving.

Quick update:

Went to NY Fri. thru Mon.  Had wallet stolen on Friday.  {{{sigh}}}  Cancelled credit cards and stopped payment on unused checks, filed police report, etc.  Oh well, coulda been worse.  

Saw Hairspray (awesome) and Avenue Q (cute).  Had one awesome meal at Tony's and fabulous cheesecake at the Brooklyn diner.  Other meals were just OK.  Weight is up a couple of pounds, but I know I'll get 'em back off within a week or so.  Felt good to get back to the gym, but can't Spin for now because of a bruised tailbone from a fall.

Thoughts on "Restriction"

Living with the band is not an exact science.  Even for experienced bandsters, "restriction" can be difficult to define, and even more difficult to determine a balance of "good" restriction.  Some people will talk about seeking the "sweet spot" (of optimal restriction) as if it's the Holy Grail of band life, but in my honest opinion, it's a myth that there's one exact amount of saline that makes for "perfect" restriction.  

I think of restriction more as an optimal *zone*, and it can vary from day to day.  Sometimes you're a little tighter, and sometimes you're a little looser, and you have to adapt your eating and behavior to accommodate those fluctuations.  So, it sort of bothers me when people post things like, "I'm wide open because I ate a whole ______ last night, should I get a fill?"  Because often the same person is posting a few days later about getting stuck!  So, *behavior* is just as important as fill level.  You don't eat a whole _______, just because you *can*.  You stick with bandster-size portions regardless.  And you don't gobble your food because you think you're "wide open", or you'll invariably have some "stuck" food, and probably PB.  

But, most of all, don't run to the doc to get a fill every time you think you're eating too much or getting hungry.  Give it at least a week (maybe two), and see how your restriction level is over that period, because getting too much saline in the band is worse than having too little.

B:  LF latte; 3/4 c. Kashi Go Lean Crunch cereal with 1/4 c. blueberries (frozen/thawed), and 1/3 c. 1% milk
S:  LF latte; 2 T peanut butter
L:  LF veggie beef soup
S:  1/2 banana w/ about tsp. peanut butter; handful Wheat Thins whole grain crackers
D:  Panera:  1/2 chicken panini (removed 1/2 of the bread), and salad
S:  2-3 bites cinnamon roll.  A friend gave it to me this morning, and I *swore* to myself I wouldn't eat it.  So, it sat on the kitchen counter all day long, calling to me.  I just ate the tender, gooey center, and...shhh...it was worth it.  The rest...down the disposal.

Exercise:  70" Spinning.  Exceeded by "March Madness" exercise challenge of 2000 minutes.  Total exercise for March:  2865"

Just posted this on the Vet Board, in response to a post about what "at goal" bandsters eat.  It pretty much summarizes my current view of food:

You know what's kinda weird?  As I get more physically fit, I've noticed a gradual shift in my thinking about food.  Eating healthy isn't about deprivation or dieting *at all* any more.  It's about being *good* to myself, and eating clean, whole, natural foods.  I'm much more in tune with my body and how it reacts to what I eat.  When I eat fatty foods, I often get a stomache upset.  When I eat a lot of carbs, I get sluggish and hungry.  When I eat lean protein, whole grains, and veggies, I feel energised, and ready to go!

Roasted Chickpeas

Someone PM'd me for this recipe.  I can't remember whether I posted it to my blog, so thought I'd add it here, too.

Roasted Chickpeas

Drain and dry a can of chick peas (garbanzo beans).  Preheat oven to 450 degrees.  Place chick peas on a rimmed cookie sheet, and place in oven.  Heat about 35-45", shaking pan to redistribute peas every 10" or so.  Cook until lightly browned and crunchy.  Remove from oven. Spray lightly with Pam, and sprinkle with seasoning.  I used garlic powder, cayenne, and oregano, but I recently bought some Tony Chatcheries Creole Seasoning with Herbs that I'm going to try the next time.
