Does your insurance company let you kno the total cost of the procedure and your total out of pocket cost of there is one? How does the whole process work once you are approved?

How long did it take to get an approval?

Maybe someone on here can help me because everytime I call Humana for an answer I get a different one. So the requirements are changing for surgery effective May 1,2017, to me this means that I would need to complete each of these requirements within 6
