Weightloss Program Requirement

This question is in regards to BCBS requirements for surgery. They stated that I'll need to have completed two years prior of 2 weightloss programs for 3 months each or 1 program for 6 months. I never done any of those kind of programs for that long before so would I have to had done them before ? Or can I start them now before submitting to insurance being that I just received those requirements?
