My ins carrier is Benefit Planners. Does anyone know what their qualifications are?

I do not need to be referred to a surgeon. I have spoken to the ins co. and been informed that all I need is to have the dr. send a letter of pre-determination and I should hear something within 5-10 working days. Does this mean that I don't have to be a certain BMI with all the other exceptions? Has anyone out there used Benefit Planners to pay for their surgery?
