Any experience with Mountain States Administration-WPPA?

Does anyone have experience with MSA-WPPA? I've searched this site to find someone who may advise me. :-( Does anyone know what I can expect from them? I am just starting the process. When I called the doc to schedule my consultation I was told to call back after talking with insurance. Insurance said that they will cover if medically necessary. Was a psych eval required by them? What type of things did you need to provide for approval? My consultation is next month and I was told that upon approval they like to have surgery dates within 1 to 1 1/2 months, so I'd like to have as much done as possible before the appointment.
