Anyone have experience with co-payments thru United HealthCare Select POS?

I have just been approved through United Healthcare Select POS after the first letter. As my policy is managed care, Bariactric Treatment Ctr was approved out of network, which means they will pay 75%. I'm estimating my cost to be about $5000.00. Does anyone have any experience with charges on an open rny - just general info, I know all cases are different.
