What documentation did you provide to prove your enrollment in weight loss program

I didn't need any documentation. I listed when, what and my approx. weight at the time. I also added everything I tried at home. Open DS, 12/17/01, no complications, down 100 lbs. in 5 1/2 months.

I didn't provide proof of anything I tried, just listed the program, or pill, indicated how long I did it, how much I lost and how much I regained afterwards. Neither my insurance company or my surgeon requested proof.

I sent to the insurance company the following... 5 Years of Medical records. 1 year of pharmacy print-outs showing me on phen and redux. 7 years of Jenny Craig records. Must have been enough, because I'm waiting on surgery July 10th now.
