Question about insurance appeal, and the end of the year

I've been denied twice by my insurance (BCBS AL XAA) for not meeting medical criteria. If I appeal again it's not likely that I'll get an answer before mid-December. How does the fact that it's the end of the year, and the fact that our plan may be substantially changed in Jan. 2004 affect my chances? Additionally, if I were approved, what happens if I can't have the surgery before the end of the year? Am I still covered in 2004? Even if it's excluded or the requirements are more strict under the 2004 policy? Once approved my surgeon doesn't schedule surgery for another month or so...Many thanks!    — Le P.
