I have Humana HMO and had no problem getting the surgery approved. My policy stated it was "only covered if medically necessary". My surgeon's office (Dr. Mark Shina in KY) had more stringent criteria to meet than the insurance company did. Insurance approved the surgery within one day of receiving the request. If your policy reads the same as mine (check under exclusions)you shouldn't have any problem. If you don't have an updated copy of your policy, all Humana Customer Service and request a copy. Good luck!
