Umm, err, I don't get it either. Are you talking about your WL surgeon needing you to search around to find examples of a letter of medical necessity for him to write and submit to your insurance company? Are you his first patient? If this is true, I'm very concerned for you on many different levels. What you're asking is very strange. Could you please clarify to see if we are reading it right? Dawn V.

I have to agree with the others. I think I'd find a new doctor. If he's going to want you to get him examples of letters, what is he going to ask you to do later that he doesn't know how??

I completely understand what you want. email me at [email protected] and I will send you mine. Everyone that has used it has been approved. dont know if that is a coincidence, but it is a good one!!! I will respond with the letter. It is in PDF format so it will have to be retyped on your docs letterhead with his license number. Deb

If I had a doctor who was going to do this life-changing surgery on me, and who didn't know how to put together a simple letter detailing my medical issues requiring this surgery, I'd run for the hills and get another doctor. Even getting a letter he could copy would have me taking my very important body to another doctor who knows what he or she is doing. This is a life-changing medical procedure and I would want only to have a very experienced doctor do mine. If this doctor doesn't know even how to write such a letter, and you go through with having him as your surgeon, and medical problems ensue following the surgery, will you again be coming back to us to help you educate him on medical procedures to save your life?

A letter of necessity should include your co-morbidities and such, explaining why you need this life saving procedure. Make sure you pick an experienced surgeon for your WLS. He or she should know how to write such letter. Good luck !!!

I just want to clear up the confusion on this question. My PCP want examples not my Bariatric Doctor. I am sure my PCP as an idea of what to write just has never done one before for WLS. He was only saying "Examples would be helpful". I appreciate all of your answers, however, the mean spirited ones are not very helpful. Thank you

I know what you mean too. I had my wls 5 years ago and I wrote up my own letter, took it to my PCP and he reviewed it, made a few additions then signed it. If I had waited for him to do it, I'd still be waiting. All the letter contained was a list of my failed weight loss attempts. All the weight loss programs you tried and failed along with dates you did them. It also listed all my cormorbidities and my family history of these same comorbidities. This may not be sufficient anymore. But that's what worked back in 2003. Joyce

Ask your WLS if he has an example. Mine does, and gives all of his patients a copy to give to their PCP so that it is done correctly. He says that he has seen too many PCPs who just aren't familiar enough with the surgery. If you haven't gotten a good example yet, contact me at [email protected] and I'll get a copy of mine to you. Good luck! You've made the best decision for the rest of your life!!
