Question:
5 yr history of MO-- any 5 yrs?

My insurance states "at least a 5 yr history supported by documentation." It doesn't say the "LAST" 5 years, and it doesn't say consequetive 5 years. I lost weight after a spinal cord injury and surgery in 2002. I wasn't dieting-- it was a side effect of my condition. I've gradually put the weight back on in the meantime--trying lots of diets trying to stall the regaining. I guess it helped a little, but here I am--back again. I have my med records for 1996-2001 showing MO. I have even more years, but was in graduate school and the student health center shreds them after 10 yrs. So when I requested them a few months ago (2007), my last time with them was 1996, so I just missed it. They're gone. Does insurance (mine is NJ BC/BS) require it to be the most recent 5 yrs or just any? Can it be cummulative? I may only have 1 weight recorded for one of those 5 yrs (2001) and they may not accept that. However, I've been MO for the last 6 months (despite my efforts to diet). I think that they want to see a history in general. They way that my office had me deal with this was by having me write a diet resume. Send me your email address and I will send you the word doc of it. Heck- use mine with your name on it if you want to!! It was a resume of ALL diet attempts and it spanned from when I was about 12 to 35 years old. Maybe this is the best way to answer their question. If the 'AT LEAST five year history...." is the question than you are MORE than satisfying it by using this format. I was approved....and am currently post-op 5 days...Keep working on it- you will get there!!

Same here. I have MA BC/BS, and when I had both my nutritional and psych consults they asked about my weights and dieting attempts for as far back as I could remember, and used all that info.

with my insurance it had to be 5 yrs in a row as well as any diet attempts that were made in that time period. Best of Luck

I have OH Anthem BC/BS. I had to write my diet history in my surgeon's office. I had no official record because I refused to let them weigh me when I went into the office. I was approved on the first submission. Good Luck!

I had to do the same thing and I am one of those women that only went to an OB and didn't have a PCP till this year. Even with my OB I dont have consecutive 5 years. I have 4/5 last years and my insurance coordinator said that would be fine. No worries!
