You can look up who your providors are on www.anthem.com. Select the anthem insurance you have then do a search within a certain mile radius of your choice from your zip code.

I, too live in Ohio and have Anthem BC/BS (POS). My surgeon, Dr. David Fallang in Middletown, Ohio (about an hour south of Columbus)was in my network and took my insurance without any problems. The gals in the office did all of the work and I was approved within 72 hours. Dr. Fallang is an incredibly experienced and competent surgeon. I highly recommend him. His number is (513) 424-7025. I live in Cincinnati, I have Anthem Premier 200 and originally was okayed to see Dr. Fallang in Middletown. However, when I submitted everything to the insurance company I was told I had to go to a surgen in the Cincinnati area. I chose Dr. Kerlakian. I was told by a representative of Anthem that this is the surgen they are using in this area from now on for all of their surgery needs.
