Question:
Anyone know what criteria BCBS Preferred Plus uses

I've checked with @ different centers for WLS. One said I needed 6 mos of medically managed weight loss and it didn't matter that my BMI was 39 because I had hypertension. The other one said my insurance required a BMI of 40 and it didn't matter even if I had co-morbidities. I'm confused, I called my insurance and they said that everyone is evaluated individually and my PCP would have to call and see if I qualify. Hi usually the criteria is 100 pounds overweight, a BMI over 40 and proof that it is medically necessary. But you best bet is to call your insurance company and ask them. each policy is different, some companies write their own policy with their own exclusions! GOOD LUCK
