Question:
What are chances of approval if doctor is in-network?

I have coverage through United Health Care of the Mid-Atlantic Choice plus plan. Under Coverage Exclusions and Limitations it states:"Medical or surgical treatment for obesity, unless otherwise specified in the covered Services Provision of the Certificate of Benefits. I have been trying to get a copy of the Certificate of Benefits, but have not received it yet. My question is that my surgeon is listed as a doctor "in-network." I have already had two appointments, one consultation, one physical. They are in the process of writing for authorization. Has anyone had approval with this kind of exclusion when the doctor was a participating physician. I am really anxious to find out.    — Jackie L. (posted on November 21, 2000)
