Question:
General Exclusions: What if a company seems to see it as cosmetic surgery?

I still don't have solid info on my insurance since my employer changed policies on me. I did gather that this is part of their policy (see below). Does that mean they won't cover, or does it mean only if it's for cosmetic they won't cover? What about medical necessity?
