Question:
I HAVE UHC CHOICE PLUSE.HAS ANYONE HAD THE SURGERY APPROVED (LAP-BAND) WITH THIS INS.

It is my understanding that UHC only covers RNY.

I have UHC but was approved for RNY. Did you call the insurance company to see was the restrictions were for coverage for WLS?

I have UHC Choice Plus and was approved for RNY with no problem. I don't know about approval for the LAP procedure.

I had lap band with UHC PPO no problems

My wife and I had uhc choice ppo. They were slow, but approved us both for lap RNY.

My UHC covers either the lap-band or the RNY, depending on the medical necessity of the patient.

I have UHC and I was approved for lapband within 3 days of submitting my info. I believe it depends on how your employer has written the policy. Some UHC policy even have and exclusion to not cover any surgical weight loss. I know this to be fact. My hubby's plan does not cover weight loss surgery. My plan does cover weight loss surgery with BMI greater than 35 with no other requirements. Make sure you find out the requirements in advance and follow them to the letter.
