Does Blue Shield HMO for CA State Employees Exclude Adj Lap Band

Does anyone know if the Blue Shield HMO plan available to employees of the State of California has a weight loss surgery exclusion? And if so, has anyone been successful in getting this insurance to cover the Adjustable Lap Band surgery?    — Victoria Lani A. (posted on September 15, 2002)
