Most insurance commissioners are located in the state capital. Most phone books have the address in the front pages, or do a browser search for the state legislature, or call any library or insurance agent. All should be able to help you. It is doubtful that he will speak wth you in person so I would advise a letter discussing your concerns. Good Luck.
