I haven't seen this exclusion yet:

My policy exclusion says: Care and treatment of obesity, weight loss, or dietary control whether or not it is, in any case, a part of the treatment plan for another sickness. My insurance had a similar provision in the booklet, it pays for no drugs, obesity treatment programs, diets, etc. Not in the book was the info that the only thing they will pay for is.... weight loss surgery! I was stunned that I was approved. Electricians union, local 134 insurance.

Your not 'asking' for treatment of obesity or weight loss. AND THAT Will be the key to your 'success' ... For WLS you are asking for surgical treatment of Morbid Obesity (NOT the same as obesity. and your excusion didnt say anything about 'surgery..... Dont rely on the customer service person...apply... IF its denied.. APPEAL.... go to 'the top'... good luck
