Question:
Documentation of diets

When you first register on this website it asks you what kinds of diets you been on if I am not mistaking mine automatically came up saying I have used Phen Phen and weight watchers. Well I need these documents but the doctor I went to for the diet pills has since retired and I did not transfer my records and this was back in 1994 and 1995 . Also I dont have proof that I was on weight watchers either because this was also a few years back too. I now this proof for Cigna for a approval could anyone tell me if there is records of this some where that I could get I am assuming there is how else would that have came up when I register. I sat down and wrote an extensive "weight loss history", which my surgeon forwarded to my insurance company. I, too, documented Weight Watchers, Nutri-System, Phen-Phen, and two other diet pills I ordered over the internet. I didn't have proof for any of it and they didn't make me present any. Have you tried just the documentation without proof or did they specifically say they want proof of what you say you've tried. I didn't need it and was concerned about the same thing and got approved. Good Luck - Sandi

I have Cigna HMO and all I did was document. I used this format: FROM TO DIET TYPE DESCRIPTION WEIGHT LOSS/GAIN Jan-89 May-89 Lo Cal College Fitness Class -60 ~160 Jun-89 May-90 ----- ----- +30 ~190 Jun-91 Dec-91 Balance Diet WIC Dietician +80 ~270 Hope this helps. I documented as far back as I could remember. If you have medical records, you can used the weights on there to help you remember where you were at that time in your life. Good Luck.

I also had CIGNA and the only thing they asked for was the diets. I gave them the name of the doctor and time frame I went to him. I also just mentioned the other diets I was on. I think the thing they liked was the fact that I provided dates.

Diet clinics open and close in California in an instant. Provide as much documentation in the form of a "diet history" and then firmly tell them records are not available. You might consider joining Weight Watchers during the approval process as a sign of your willingness to comply. WW is a medically developed program. good luck
