Question:
I was wondering if anyone had a sample appeal letter that I might go by?

My insurance has an exclusion in it and I am going to have to appeal even though I have a letter from my primary doctor saying he thinks it will benefit me and that I am a good candidate for the surgery...Any suggests or samples of letters would greatly help me....Thanks in advance, Christi    — butterflyeasttx (posted on April 4, 2002)

Click on "library" at the top of the page and type in "appeal letter." You'll find tons of them.
