What requirements does United Healthcare POS need to approve?

My United Healthcare POS policy didn't require any special testing or a phych exam. My surgeon, on the other hand did require both. I found them to be very helpful on the phone. You can call them up and ask them what your particular policy requires, if anything...Good Luck to you ---->>
