i work at an insurance company and we get request like this all the time.
if you call your doctor's office, they should be able to give this to you.
if they refuse, you may want to try and call your insurance company. we
normally have general list and can find the procedure code through our
books. i will research tomorrow at work and see if i can find it. keep
trying. kim
