what kind of copay for bpd/ds on ohio med and what do you need for preauth?

I really depends on the policy you have. I had the DS with UHC and was required to pay my $10 copay for the entire procedure. No preauthorization was required. Just call your insurance company and ask them about your specific policy.. Good luck to you!!
