EBMS, BC/BS, Help!! Need Info!

Hi, my husband's insurance changed last October 1 month before I was due for surgery. It went from PHCS to BC/BS Carefirst. We cancelled surgery with a doc down in San Antonio b/c we decided to move to Indy so he could work up here. So I went with a new surgeon, and my husband's company thought BCBS would cover the surgery right away since we had a pre-cert letter from the former company. Now, 4 months later, they (BCBS) still have not made a decision on if I can have the surgery or not. Here's the thing, my husband is tired of his current job, it's stressing him so much and he rarely has time to see us, his family. So he found a new job but the insurance there is Employee Benefits Management Service. An insurance I know nothing about and can't seem to find on the internet :( Anyone had them or know if they'd cover? We're also looking to buy an individiual family plan thru BC/BS Anthem locally and we're wondering if they'd approve the surgery based on information from the PHCS approval or not. Anyone had this type of thing happen to them? I'm going nuts wanting this surgery but I don't feel it's right for me to make my husband stay with a company that treats him like crap.
