I Live in Central Texas and purchased Insurance under my side business name
through Scott & White. I had absolutely no problem getting approved
through Scott & White. The entire process can take up to 6 months -
but well worth the wait! Good Luck!

I live in Michigan and we are self-employed. I got an individual policy
through BC/BS(traditional blue). They will pay. I gave them the surgery
code and diagnosis code and said yes it is covered, but a possible 6 month
waiting period for pre-existing condition. I'm going through Bariatric
Treatment Centers. They do everything for you insurance related. I called
a person at BC/BS directly and she was really wishy-washy about it, until I
gave her the surgery code. (BTC provided that code for me) You should
check into BC/BS again, because an individual policy is expensive. I pay
$215.00 a month. The surgery code is #43846. The diagnosis code is #27801
or 27800. Hope this helps you.
