Question:
Has anyone had surgery approved with United Medical Reasources/Tompkins Industries

I have not seen surgeon yet, but was checking my coverage and got this reply from the insurance... Morbid Obesity is an exclusion under your plan unless your doctor submits a pre-determination for the surgery. The pre-determination must include a full cardiac evaluation, psychological evaluation, initial history and consultation from provider. I am not sure what the initial history and consultation mean and whether it can come from the surgeon or PCP. Has anyone been approved by UMR/Tompkins?    — jaylex08 (posted on April 24, 2005)

This is pretty much the same as any insurance. The initial history is your weight history for the past 5 years. The consultation is with your surgeon. If you have to go through a diet program that will be through your PCP.
