Question:
How do i get approval from my insurance, if PCP kept no records

MY PCP NEVER KEPT RECORDS OF MY WEIGHT LOSS ATTEMPTS. HOW THAT I'M TRYING TO HAVE THIS SURGERY(GASTRIC BYPASS). IF INSURANCE ASK PAST WEIGHT LOSS ATTEMPTS, I DON'T HAVE ANY. MY PCP IS NOT FOR THIS SURGERY, SHE KEEP TRYING TO TALK ME OUT OF IT. I HAVE PROBLEMS WITH MY BACK & KNEES. I HAVE NO OTHER HEALTH PROBLEMS, BESIDE BEING MORBID OBESITY. Have your PCP and the bariatric surgeon bothe write letters emphasizing the need for the surgery to help your knee conditions and also for prevention of early death. Every insurance company is different but with a BMI=62 that should be enough.
