Has anyone been approved by Private Health Care Systems?

I have PHCS and will be seeing a surgeon today. I called them last week and they said that the criteria is TWICE your ideal weight, not just 100lbs. They said that they usually approve if the doctor feels it is medically necessary. By looking in the insurers section for VA, I found a few people in my state who have also been approved by PHCS. You should check the insurers list for your state.
