Question:
I need to write a letter to my insurance company to help with approval

I have had three doctor supervised diets in the last 6 years. Two of those doctors conviently lost my medical records, so my surgeons office suggested i write a letter to my insurance company, stating how long ive been overweight etc etc any body out there have a format to follow. I was denied after my first try. My appeal was denied as well. Later I found out why, not enough diet history!!! I was very upset to find this out because when I went to the surgeron office they had me filling out a 20 page packet in 1/2 hour. They were suppose to mail the packet prior to the appointment to give me time to fill it out. I changed pcps and five months later my Dr. ask me if I thought of wls, boy did my eyes light up!!! I told yes and that I was denied and why. So she told me that the Medical review board is meeting and to give her my COMPLETE diet history. Well she went to the meeting with my diet history and spoke to them about me and they approved my surgery right then and there!!! I couldn't believe it!!!! They never question what I wrote or verified ANYTHING. If your pcp is understanding and got alot of heart I would suggest that you ask him or her to personally approach the Medical Review board the day they review all requests. When you fill out the packet add your own diet history on a separte piece of paper. Good Luck to you!! I got more to say about this but prefer to tell you one on one. If you get my drift.
