Question:
I need a copy of a referal for consultation letter from PCP.

I was able to get a consultation appt for one week after I called to schedule. My insurance is a PPO and does not require a referal for consultation. I just got a letter yesterday from the surgeons office stating that I will need to ask my PCP for a referal for "consult for obesity". I contacted my Dr.'s office and they said that due to the short notice (consult is on Tuesday) that I can type up a referal letter and take it in for the Dr. to sign. Gee, they usually just have a simple form in the office that consists of a couple of lines. The ones I have seen anyway. You probably need to just put: PCP-Name Referred to-Name Reason- Consult for evaluation regarding .........(whatever) Date-..... Signature (PCP)........ Hope this may help!
