You can go to Dr.Macura(718-351-3232)in staten island,I am going to Dr.Gadaleta in Manhasset Long Island (516-482-8657) I am scheduled for surgery on dec 15th. Also Dr.Gagner(212-241-5339,3336) I don't think he takes your insc.Hope this helps
