I was insured by BC/BS of California and got approval the same day they received the letter from my surgeon.

I have BCBS of CA and they approved within 5 days for a BPD/DS. Would have been sooner for the RNY but they had to have my surgeon talk to theirs.

Just found out yesterday that BC/BS approved mine in about 6 working days. Actually, my PCP's medical group for the HMO approved it. HMOIL approves the surgery as long as it's medically necessary, and they basically approved it on the phone as soon as my surgeon asked; it's the medical group that says yes or no (and it's very easy to change groups if you don't mind changing drs).
