Question:
Who is responsible for putting the info together?
After two denials I was told that if I could "verify" the co-morbidities i listed in my appeal letter I would be approved. I signed a release form to have the info sent to my surgeons office but I my PCP moved to a different state so it seems like the burden of proof is on me! Isn't there any way you can contact your PCP, even if they moved? I would
think he/she would be required to keep the records, or at least give turn
them over to a replacing physician. Might want to check into that first
off. Your PCP could not just throw them away. Good luck.

I worked in a medical facility for several years. Your PCP is required to
keep medical records. You may have to pay a nominal fee ($10-$15) but you
should be able to get a copy of your medical records. I have kept a large
binder of my medical records for several years. I even have sections for
insurnace correspondence. That really assisted me with obtaining approval
for my RNY due to take place next week. I would advise everyone to keep an
updated book of medical records especially during the approval process.

I have found information concerning Doctors,(addresses, schooling, etc) on
the AMA web site. If he is a member, you should be able to locate him
through this. Someone has to keep your records. I'm surprised he didn't
offer to forward them to your new doctor.

Your PCP must keep your records for 7 yrs. You should be able to find him
via the internet. I make sure I have copies of all my medical records
now...you just never know when you'll need them.
